Life Care Planning is an integrated approach that addresses the health care, legal, and financial issues of aging and disability.

Why should you consider Life Care Planning? A Life Care Plan promotes good health, safety, and well-being at all times. The Life Care Plan assists with health care and long-term decision making for the loved ones. The plan can identify and assess long-term care including whether to remain in the home or live outside of the home. Financially, the Plan works to identify all potential sources of payment for health care and to attain eligibility for public benefits programs (SSDI, Medicaid, VA, etc.) To protect family wealth



The Elder Law Firms provides the following services in a Life Care Plan.

We will consult with you in drafting and implementing your Life Care Plan. Your plan includes all necessary legal documents, such as wills, powers of attorney, trusts, etc. After the plan is complete and the documents prepared, The Elder Law Firm remains available to assist in asset restructuring which includes conferences, research and telephone calls necessary to implement and monitor your Plan. At the appropriate time our firm will file applications for SSDI, VA, and Medicaid benefits. In addition our firm is available to complete annual reviews of all benefits being received and to modify your Life Care Plan when laws change.

The Elder Law Firm can make an initial assessment in your home, residential or health care facility, if needed. We can assist in obtaining services of a Geriatric Care Manager (GCM) or geriatric licensed social worker (GLSW) to assist clients. Other professionals may be associated to with development and implementation your Health Care Plan and continued monitoring of the Health Care Plan. We can assist with living arrangements and placement. Our firm may help coordinate locating all available community resources.

Geriatric Planning

Geriatric planning helps meet critical needs for senior citizens. Geriatric care management happens during times of crises in the family. However, many preventive measures can be used prior to reaching crisis situations.
Geriatric care managers assist people who are in need of help from others for day-to-day tasks, financial affairs, and coordination of health care services. They solve problems by using social work and nursing tools, including client assessments, care planning, referrals to other resources, government benefits coordination, and continuous monitoring. Geriatric care managers are frequently licensed social workers or registered nurses.

The Elder Law Firm will help locate geriatric care management services to relieve our life care planning clients and families from the emotional stress of being caregivers by saving them time and money by being able to prevent crises.

Patients Advocates

When people live in assisted living facilities and nursing homes they have certain rights. At times these rights may be violated by the facility where they live.

If so, the resident may file a formal complaint against the facility. The first place to begin a complaint is with the local Ombudsman. The facility should the name and contact information for the Ombudsman.

If a resident’s complaint is not satisfied, a hearing may be requested. An administrative law judge will hold a hearing where evidence of the complaint will be presented. The judge will then determine if the complaint is justified and what action is necessary to rectify the complaint. A person may appeal the judge’s decisi
